Peptides that promote growth hormone release use the ability of the pituitary gland to release its own endogenous GH, instead of administering exogenous GH directly into the body. There is a difference in not only the method of action but also in their legal status. Here, we provide a summary of the literature available regarding Ipamorelin, Tesamorelin and similar peptides, discuss the various products available in the market and give an overview of the differences between a research peptide and an FDA-approved one.

How Growth Hormone Peptides Work

GHRH Analogues vs GH Secretagogues

The release of growth hormone from the pituitary gland is regulated by two main factors: growth hormone-releasing hormone (GHRH) and ghrelin. The analogs of GHRH, including Tesamorelin, act on the same receptor and imitate the effect of GHRH in the body. The substances known as GH secretagogues, such as Ipamorelin, have an ability to act on the ghrelin receptors (growth hormone secretagogue receptors or GHSRs). Both are effective in increasing the amount of the growth hormone produced, but usually in different ways. The third group includes GH fragments such as AOD 9604 which serve to produce the desired results with respect to elicit only particular effects.

The GH Axis in Published Research

The hormonal hypothalamic-pituitary GH axis is an endocrinological pathway clearly described. The action of the hypothalamic GHRH causes the secretion of GH while that of somatostatin inhibits it. Released GH acts on the liver, resulting in the production of IGF-1 that initiates many effects of GH in the tissues. The goal of published peptides is to make this axis work on the input level (inducing more GH secretion) rather than supplying the body with GH directly, which preserves the native pulsatile nature of secretion and feedback regulation of the organism.

Ipamorelin: Selectivity and Study Data

Published Efficacy Data

Ipamorelin is a pentapeptide (5 amino acids) that stimulates GH release through the GHSR without significantly affecting cortisol or prolactin levels. This selectivity distinguishes it from older GH secretagogues like GHRP-6, which can stimulate appetite and raise cortisol. Published Phase II clinical data on Ipamorelin demonstrated dose-dependent GH release in human subjects with favorable tolerability. A 1998 study in the European Journal of Endocrinology characterized Ipamorelin's selective GH-releasing properties in human volunteers.

Side Effect Profile

As stated in the published Ipamorelin data reports, there are mild side effects that are in line with increased GH levels: temporary water retention, occasional stiffness in the joints at high doses and slight reactions at the site of injection. These effects are pharmacological (caused by increased GH and IGF-1 levels) rather than toxic effects related to the peptide per se. So far, there has been no report of any organ toxicity that can be attributed to the Ipamorelin peptide at the usual research doses. There is little information on the long-term safety data from clinical human trials.

Tesamorelin: The Only FDA-Context GHRH Analogue

Approved Clinical Use

Tesamorelin (trade name Egrifta) has been approved by the FDA for the reduction of excess abdominal fat in HIV-related lipodystrophy. A synthetic analogue of GHRH composed of 44 amino acids, Tesamorelin is delivered via subcutaneous injection at a daily dose of 2 mg. The FDA approval was granted due to the results of Phase III trials that demonstrated a significant reduction in visceral fat compared with placebo. Tesamorelin is, so the only peptide that stimulates the production of growth hormone in this list with the completion of Phase III trials and regulatory approval.

Research Beyond Lipodystrophy

Published research on Tesamorelin extends beyond its approved indication. Studies have examined its effects on liver fat reduction in non-alcoholic fatty liver disease (NAFLD), cognitive function in aging adults and body composition in non-HIV populations. A 2019 study in The Lancet HIV evaluated Tesamorelin for hepatic steatosis in HIV-infected patients and reported significant reductions in liver fat. These off-label research directions are active areas of investigation.

Ipamorelin + Tesamorelin Stack

Rationale for Combining

The justification behind the combination of Ipamorelin and Tesamorelin lies in dual-pathway stimulation: Tesamorelin effects activation of the GHRH receptor while Ipamorelin stimulates the ghrelin receptor. Research literature on both agents points to the possibility that administering both substances could lead to an enhanced GH release effect in excess of that coming from their independent use. There are compounding pharmacies providing this combination, and Real Peptides produces a ready-to-use Tesamorelin-Ipamorelin compound, although up to now only the available research hasn’t involved direct clinical studies of the stack versus each agent’s separate use.

Available Blend Options

Real Peptides has a product Tesamorelin and a Tesamorelin-Ipamorelin mix with lot-specific COA documents while PeptideGod offers Ipamorelin alone and AOD 9604, but it does not have Tesamorelin at the moment. If you are searching for a vendor for both compounds with COA documentation, then Real Peptides is the primary choice available in the directory.

FAQ, Growth Hormone Peptides

What is the difference between Ipamorelin and Tesamorelin?

Ipamorelin is a growth hormone releasing peptide made up of 5 amino acids that binds with the ghrelin receptor to stimulate the secretion of growth hormone. Tesamorelin is a peptide consisting of 44 amino acids that acts at the growth hormone releasing hormone receptor, which stimulates the release of growth hormone. Hence both peptides stimulate growth hormone production but through different mechanisms of action. Tesamorelin has been approved by FDA for treatment of HIV related lipodystrophy, while Ipamorelin has undergone clinical trials but is not FDA approved.

Is Tesamorelin FDA-approved?

Yes. Tesamorelin is approved by the FDA as Egrifta for the purpose of reducing excess abdominal fat in people with AIDS-related lipodystrophy who have excess fat in the abdomen. It is given as one injection of 2 mg per day under the skin. However, this approval is not applicable for other uses.

Do growth hormone peptides have side effects?

Published data reports side effects consistent with elevated GH/IGF-1 levels: water retention, joint stiffness, temporary numbness or tingling and injection site reactions. These are dose-dependent and pharmacological rather than toxic. Tesamorelin's FDA label provides the most comprehensive side effect data in this category. See our side effects guide for full details.

Can GH peptides be stacked?

Ipamorelin and Tesamorelin are usually combined due to the fact that their respective target receptors are not the same. A good example of this is Real Peptides, where the Tesamorelin-Ipamorelin combination is available pre-mixed. Currently, there are no clinical studies demonstrating significant benefits of the combination over the separate use of either peptide.
